application for a New Well, a repair or an abandonment of an existing
well. The fee for an application for a new well is $75.00. There is no
fee for a repair or abandonment. An Environmental Health Specialist will
evaluate the property to determine whether or not the lot will support a
private well water system in its present condition. The Fee for a New
Well Permit is $200.00.
On-Site Wastewater Application [complete application on-line] Applicant fills out an application and makes an on-line payment by
using a VISA, MasterCard, or American Express credit card. Approved Credit Card Transaction
waives an actual application signature. This will authorize an Environmental Health Specialist
to evaluate the property to determine whether
or not the lot will support a septic tank system in its present
condition

evaluation results by parcel Number] After the application and site evaluation are completed, you will
receive a letter describing the results of the evaluation whether or not
the lot will support a septic tank system in its present condition.
Recommendations for necessary site improvements may be made.
Authorization for Wastewater

[Parcel Number or Permit Number] Proof of an approved water source (i.e. an approved well or a copy
of your receipt from the water department ) and the improvement permit
with 2 sets of house plans must be submitted.All site modifications that are listed on
the improvement permit must be completed.
A sealed sewer line from the building
to the tank .
A constructed driveway
An approved and functional water
supply.
The "as built" building conforms to
submitted plans.
Alternative or Innovative Systems
In addition to the items needed for a
conventional system, the Health Department requires a letter from the
engineer stating the system was installed as designed.
The Health Department also requires a
copy of a contract signed by a certified operator and the owner or
owners representative for maintenance of the system.
